So the main character, Tsuna, is basically this guy who struggles in school and isn't very good at sports. Everyone calls him "No good Tsuna." One day he comes home to find his mother has hired him a home tutor. The tutor comes, and looks like a little kid, mainly due to his height. The tutor, Reborn, tells Tsuna that he has been sent by a mob boss to train Tsuna to become the next mob boss of the Vongola family. Every other descendent of the Vongola family has died. Later, when Tsuna is walking to school with Reborn, they see the girl Tsuna likes, Kyoko. Reborn can see Tsuna's feelings, and promptly shoots him in the face. Apparently the bullet that he shoots Tsuna with is called the "dying will bullet" and makes the person shot much stronger in order to achieve what they were regretting at the time of death (in this case, Tsuna not telling Kyoko that he likes her). If they are not regretting something when they are shot they will actually die. Tsuna, in dying will mode, goes to tell Kyoko how he feels, except she runs away, as Tsuna is only in his underwear after awakening in dying will mode. Later, an upperclassman who takes offense to Tsuna's weird actions towards Kyoko challenges Tsuna to a kendo fight. Tsuna doesn't want to, but is shot by Reborn and is suddenly capable of beating up the senpai. (I think dying will mode lasts for 5 minutes?) Thus, other students begin to think that he might not be as much of a loser as they thought.
And that's pretty much what happens in the first chapter!
